     49-4-303. Applications for permit. (1) Applications for a special parking permit may be made to the department of justice on forms provided by the department that require sufficient information to determine eligibility for a permit. The application must be accompanied by:
     (a) a certificate from a licensed physician or a licensed chiropractor describing the extent of the applicant's disability; and
     (b) a fee of $1.
     (2) Applications must be available at the office of the county treasurer in each county and directly from the department.
